You can learn about what your chiropractic benefits are by
calling your insurer. There should be a telephone number on your insurance
card. To understand what benefits you are entitled to, ask the following
questions:

Does this plan pay for chiropractic treatment?

Does it pay for examinations, x-rays or physical therapy?

Can I go to any chiropractor or do I need a referral?

For HMO/PPO Plans: Do I need to go to a chiropractor who is in this network?

For HMO/PPO Plans: Do I have *Out of Network Benefits?

Do I have to pay a yearly deductible?

Do I have a co-pay or percentage to pay?

What does the plan pay per office visit?

Is there a limit to the number of treatments I can have?

Are there any other limitations I should know about?
Note: Some plans require pre-authorization. This means
that you need to call your insurer and notify them that you will be seeing a
chiropractor. You will then be given a pre-authorization number, and you
should give that number to our insurance staff.
*Out of Network Benefits: This means that you may receive care
outside your network. If you have this option, there may be a yearly
deductible and co-pay. You should be clear about what those are.
After you call your insurance company, if you feel that you still don't fully
understand your benefits, our insurance staff will be happy to assist you.
